Common questions

What you need to know.

Common signs include persistent fatigue, low libido, difficulty building or keeping muscle, mood changes, brain fog, and increased body fat. Levels decline roughly 1 to 2 percent per year after age 30. The only way to know is lab work — which is why a baseline TRT panel is required before treatment.
A baseline panel including total testosterone, hematocrit, LH, and PSA is required before any prescription, with follow-up labs at 90 days, then annually, to check that levels are within the normal range and to monitor safety markers. Your panel is fulfilled at a CLIA-certified lab near you. Lab costs are billed separately.
Enclomiphene stimulates your body’s own testosterone production rather than replacing it, which preserves natural function and fertility. It is often the first choice for younger men or those planning to have children. Direct TRT typically produces larger, more reliable increases.
Direct testosterone replacement suppresses natural production and can reduce sperm count while on therapy. Men who want to preserve fertility may be offered enclomiphene instead, or hCG alongside TRT. Discuss family plans with your provider before starting.
Dosing starts conservatively and is adjusted based on follow-up labs and symptom response. Hematocrit, PSA, and estradiol are monitored over time, and medications like anastrozole can be added if estrogen conversion needs managing.
Injections are the most common and cost-effective route, typically once or twice weekly. Topical cream is a daily, needle-free alternative. Your provider will recommend the route that fits your levels, lifestyle, and preference.
