Two fundamentally different strategies
TRT supplies testosterone directly — injection or daily cream — producing reliable, well-studied increases. Enclomiphene takes the indirect route: it blocks estrogen feedback at the pituitary, prompting your body to raise its own production through LH and FSH signaling.
TRT
Direct replacement · largest, most reliable increases · suppresses natural production & sperm count · weekly injection or daily cream
Enclomiphene
Stimulates own production · generally less suppressive of sperm production · not FDA-approved · oral daily tablet · gains depend on pituitary response
The deciding factors
- Fertility plans → this is usually decisive. TRT suppresses sperm production while on therapy; enclomiphene preserves it. (HCG added to TRT is a middle path.)
- Age and baseline → younger men with intact pituitary signaling often respond well to enclomiphene; men with primary low production usually need replacement
- Magnitude → if levels are very low or symptoms severe, TRT’s reliability typically wins
- Preference → daily oral tablet vs weekly injection is a real lifestyle difference
Either way, the protocol structure is identical and non-negotiable: baseline labs first, conservative start, re-test at 90 days, ongoing monitoring. The medication is a choice; the monitoring isn’t.
Labs required
Your labs decide.
